
POLICIES
Enrollment Acceptance Policy:
Be 18 years of age or above. Submit a copy of either a High School Diploma or GED, or an essay of equivalent life experience, of 3 to 5 typed double spaced pages. Submit a completed Application form, Enrollment Agreement, and all fees applicable. Agreement valid upon receipt of completed agreement and letter of acceptance sent to student by the MIM school administration.
Occupational Training:
This program is intended to prepare you to enter the field of Massage Therapy. Contact your state and local area to determine the requirements necessary to obtain licensure. This information must be obtained before you commit to this program to ensure you will meet your state's requirements. MIM is not responsible to determine state/local area requirements for the student, nor is it liable if the student does not meet said requirements.
Student Insurance:
Each individual student must carry liability insurance during the enrolled period in MIM's Massage Therapy Program. MIM works with different insurance program s if you need assistance. Send proof of insurance coverage to MIM for your student file.
Average Completion Times:
The course is designed to take 12 months to complete, although MIM allows you up to a maximum of 1.5 times the designated length of course for completion (i.e., 18 months). Many students are able to complete their course in a shorter period of time than the average of 12 months. Obviously, the more time you dedicate to studying the faster you will complete the course.
Leave of Absence, Reinstatement:
Students who wish to take a leave of absence or students who have been inactive for over 6 months, need to petition MIM in writing explaining the necessity for a Leave of Absence. Once the leave of absence has expired, a student interview will be arranged and reinstatement or termination will be determined at that time. A Reinstatement fee of $100.00 will be assessed for each six month period up to 1 year to complete the program.
Cause for Termination:
Once the maximum time (i.e., 18 months) on the program has passed, you will be terminated from MIM's Distance Education Program. Other causes: expiration of theory hold, failure to pay, or inappropriate behavior. Reinstatement is available case by case.
Student Conduct:
Students who show a pattern of absences, violate MIM regulations or polices, interfere with another student or instructor, steal or destroy property, are boisterous, obscene or in any other way conduct themselves in an unprofessional manner (as deemed by the Administration) are subject to immediate Termination.
Shipping Outside the United States:
Additional shipping costs must be paid by the student if they live outside the continental United States.
Individual Success:
The staff at MIM is committed to assisting all students to be their very best, but the student determines the level of success. Your success or satisfaction is dependent upon your individual efforts, abilities and application of yourself to the requirements of the school.
Transfer of Credits:
MIM accepts credit transfers on individual review process. MIM makes no guarantee of credit transfer either to or from this institution. Acceptance of credits from another institution is at the sole discretion of each receiving institution. MIM does not imply, promise, or guarantee transferability of credits earned at any other institution.
Placement Assistance:
MIM does not provide placement assistance for graduates of the program due to the tremendously large geographical area covered.
Failure to Pay:
Failure to pay any amount due in accordance with your Time Pay Agreement, will result in assessed late fees and/or accruing interest on any outstanding loans or balances. The school certificate, final grades, references, transcripts and all services are suspended from the student until such time as the payment of all tuition and fees is brought current or paid in full.
Cancellation & Refund:
Should you request cancellation of this agreement, the following cancellation policy shall apply: (1) If MIM is notified of cancellation within 3 business days after the enrollment agreement has been signed, you will be refunded all tuition paid to the school, including the nonrefundable registration fee of $100.00. (2) After the 3 day cooling off period, you shall be refunded, within 30 days of your discontinuing, a prorated percentage of all tuition paid over a $100.00 non-refundable registration fee.*: (a) 100% of the balance of tuition through completion of 20% of the term (11 weeks); * (b) 50% of the balance through completion of 30% of the term (16 weeks); (c) no refund after 16 weeks.*. All cancellations must be submitted to MIM in writing, any refunds due will be calculated from the date of the post mark.
*All materials must be returned in original and resalable condition or the retail cost of the materials will be deducted from the refund. Non negotiable.
This agreement is a legal and binding contract between the student and Myotherapy Institute of Massage. No other agreements, oral, written, expressed or implied can be accepted by the school. |
